Brutto is ugly, cattivo is bad. It gets confusing in the translation especially given that the characters are introduced according to the Italian words. And also because you can argue that they are all "bad" in some way, and that Lee van Cleef is ugly
In other words the direct translation is "The GOod, The Ugly and THe Bad"
But Wallach's autobiography leaves no doubt - it was called "The Good, The Bad, And Me"
