You could have a look at this book for the general endurance fitness needed for altitude.  For the climbing – doing it?

The uphill athlete website is also a good place to start looking

In reply to Hadsbjerv: the Big Mountain training program at Mountain Tactical Institue is a really good 12 week fitness program for developing a good ‘mountain chassis’. I’ve used this program several times in the past.

The hardest part is remembering how to do all the exercises they have. Usefully they have a section with descriptions and in most cases videos on how to do them.

Absolutely amazed no-one has yet mentioned the Bible:

"In Training for the New Alpinism, Steve House, world-class climber and Patagonia ambassador, and Scott Johnston, coach of U.S. National Champions and World Cup Nordic Skiers, translate training theory into practice to allow you to coach yourself to any mountaineering goal."
